ХРОНОБИОЛОГИЧЕСКИЕ ПОДХОДЫ К КОМБИНИРОВАННОЙ ЛАЗЕРНОЙ ТЕРАПИИ В УРОЛОГИИ

После того как было доказано, что первичным механизмом биологического (терапевтического) действия низкоинтенсивного лазерного излучения (НИЛИ) является термодинамический запуск Ca2+-зависимых процессов [С.В. Москвин, 2008], а инициирование вторичных процессов биорегулирования происходит под влиянием распространяющихся волн повышенной концентрации Ca2+ с полупериодом, равным 100 с, появилась возможность оптимизации временных параметров лазерного воздействия. Кроме того, стали понятны и принципы сочетания различных действующих лечебных факторов. Хронобиологические принципы успешно реализованы в лазерном физиотерапевтическом комплексе «Матрикс-Уролог». В методике локального лазерного отрицательного давления (при эректильной дисфункции и простатитах) время повышения и снижения отрицательного давления ограничено 100 с, смена излучателей (длины волны) осуществляется через этот же период, модуляция НИЛИ осуществляется с частотами 10 Гц (максимальная вазодилатация) и 40 Гц (максимальная эффективность высвобождения Ca2+ из внутриклеточного депо). Время процедуры при воздействии вибромагнитолазерной головкой ВМЛГ-10 (при простатитах) также ограничено 5 мин. Понимание физиологии происходящих под действием НИЛИ в биотканях процессов позволяет синхронизировать с ними частотные и временные параметры методики, что позволяет значительно повысить эффективность метода, получать стабильные и воспроизводимые результаты лечения.
CHRONOBIOLOGICAL APPROACHES TO COMBINED LASER THERAPY IN UROLOGY

After which it was proved that as a primary mechanism of biological (therapeutic) action of low level laser radiation (LLLR) is a thermodynamical start of Ca2+ independent processes [S.V. Moskvin, 2008] and the initiation of the secondary processes of bioregulation occurs under influence of the transmitted waves of the increased Ca2+ concentration with half period equaled to 100 s, arose the possibility to optimize the temporally parameters of laser action. Moreover the principles combining different therapeutic factors came as well to be understood. Chronobiological principles are successfully realized in the laser physiotherapeutic complex “Matrix-Urolog”. In the methods of local laser negative pressure (erectile dysfunction and prostatitis) the time of the negative pressure increasing and negative pressure decreasing is limited to 100 sec, changing of the radiant (wavelength) occurs through the same period, LLLR modulation is exercised with 10 Hz frequency (maximal vasodilatation) and 40 Hz frequency (maximal efficiency of the Ca2+ release from the intracellular depot). The time of procedure under the influence of vibromagnetlaser head VMLG-10 (prostatitis) is limited as well to 5 min. Understanding of physiology of the processes arising under the LLLR action in the biotissues enables to synchronize with them the frequency and time parameters of the methods which enables significantly to increase the method efficiency, receive stable and reproducible treatment issues.
